Responsibilities
PART TIME HHA/CNA- 3rd Shift (11pm-7am)
The HHA or CNA is responsible for providing personal care and service to an intimate group of Alzheimer / Dementia residents (Fieldstone Memory Care). The HHA or CNA provides care and program/activity needs to residents while assisting in maintaining a positive physical, nutritional and social environment, while ensuring they feel safe and purposeful. This position is Third Shift (11 PM- 7 AM), Monday-Friday, 24 hours per week

• The CNA/HHA Assists residents with activities of daily living, including meals, bathing, dressing, grooming, toileting, transfer, mobility, and incontinence care
• Observes and reports any changes in resident's condition (behavior or health), takes action to ensure resident safety
• Demonstrates knowledge of residents relative to participation needs, physical abilities, interests, cognitive capacity and attention span capability
• Initiates programming and activities with residents, leading one-on-one, and group activities
• Provides appropriate interventions to maximize the resident's social and Activities of Daily Living (ADL) functioning
• Provides compansionship and engagement activities
• Assures shift information is clearly communicated to peers and supervisors
• Accurately assesses, communicates, and documents residents' status
• Supports the resident's family and other members of the caregivers team, reports any resident or family concerns
• Assists residents with medication per state regulations
• Participates in service plan reviews

Qualifications

• Must be a Certified Nursing Assistant (CNA) or Home Health Aide (HHA)
• High School diploma or GED required
• Must be nurturing, caring, and reliable
• Ability to read and write the english language
• Dementia experience preferred
