We operate in Los Angeles and community East and South East of Los Angeles County.

JOB TITLE: SPA 4 Transitional Housing Case Manager

We provide stable housing and other community based services that create opportunities for them to lead productive lives.

The primary responsibility of the case manager is to provide case management services to youth age 18-24 that are enrolled in the program. Eligible individuals will be referred by the SPA 4 Coordinated Entry System. The goal of the program is to transition youth in to permanent housing opportunities. The facility is located in the Pico Union area just West of Downtown Los Angeles.

The case manager will be involved in coordinating activities with other departments such as asset management and emergency housing services under the supervision of the SPA 4 Short-Term Housing Supervisor. From time to time, the case manager may be asked by program manager or executive staff to conduct activities or tasks necessary to the operations of the organization.

DUTIES AND RESPONSIBILITIES

Under the supervision of the SPA 4 Short-Term Housing Supervisor, the position will:

  • Coordinate intake of eligible clients.
  • Assess clients’ challenges and barriers as well as strengths.
  • Elaborate with client his Individualized Service Plan that will be updated monthly.
  • Provide intensive case management services to achieve the outcomes set in the ISP.
  • Provide clients with referrals and monitor progress as needed, in particular to mental health and substance abuse services, and toward opportunities for educational & career development and personal enrichment.
  • Evaluate transportation needs and support them in developing a sustainable plan.
  • Provide clear direction, support and follow up to clients.
  • Support clients in identifying permanent housing options.
  • Provide support to clients as needed to deal with any issues related to housing retention.
  • Maintain files in a secure and locked cabinet.
  • Clarity training and access to input confidential client data into the HMIS system on an ongoing daily-to-weekly basis where only case manager manages account
  • Collection of client documentation, input notes in Clarity HMIS system based on documents and maintain in client’s file in an organized and structured manner
  • Prepare and submit reports in a timely manner.
  • Follow the agency professional code of conduct at all times.
  • Develop appropriate procedures in case of crisis.
  • Coordinate other program staff in service delivery and facility monitoring.
  • Delegate activities and follow up to other program staff to maximize clients’ progress.
  • Under the supervision of SPA 4 Short-Term Housing Supervisor, ensure that scheduling of all staff in the program is consistent.
  • Under the supervision of SPA 4 Short Term Housing Supervisor, implement and/or develop, as needed, policies and procedure for all aspects of the program.
  • Other duties and responsibilities may be developed by immediate supervisor according to program needs.

  • Physical Skills and Abilities:

  • Ability to talk or hear in order to give and receive information and instructions
  • Ability to stand, walk, sit, use hands to finger, handle or feel objects, tools or controls
  • Ability to reach with hands and arms
  • Ability to use computer keyboard up to 50% of the day
  • Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment
  • Ability to lift and/or move up to 20 pounds

    Equipment and Computer Software Skills and Abilities:

  • Computer literate (basic working knowledge of Microsoft WORD, Excel, Outlook, and PowerPoint)
  • Able to use telephone, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required
  • EDUCATION, EXPERIENCE, CERTIFICATES & TRAINING

  • Bachelor’s degree or 4 years of experience in a relevant setting;
  • Experience working with transitional aged youth, individuals with mental disabilities, low income families.
  • Demonstrated ability to work with a culturally diverse clientele & staff
  • Bilingual and fluent in English and Spanish a must
  • Valid California driver’s license and personal vehicle insurance carrier relating to clients, staff agency representatives, community members and others.
